PaulaPaula Bausman, MJ
National Account Services Manager
Paula’s career started as a Registered Dietitian, consulting at long-term care facilities. During her time as an RDN, she specialized in assessing residents at high nutritional risk. Having a desire to impact the quality of care in nursing homes, Paula became a care surveyor with the State of Illinois, served on the state training faculty team, and acted as a member of the Education Committee for the Illinois Pioneer Coalition. Paula joined the abaqis training team upon the completion of her degree, a Master of Jurisprudence in Health Law from Loyola Law School. Additionally, Paula has obtained Certification in Healthcare Compliance (CHC) and Lean Leadership accreditation. Passionate about resident-centered care, Paula is proud to be part of Providigm’s mission to improve the care of the elderly.

PatrickPatrick Campbell, RN, BSN
Senior National Account Services Director

Patrick is a registered nurse with experience as an assistant director of nursing and director of nursing in long-term care. He spent over six years as a state surveyor in North Carolina, serving as a team leader and as a frequent member of the state’s informal dispute resolution committee. Patrick was selected as one of the original QIS trainers for North Carolina and became a Master QIS Trainer after joining Providigm in 2010. He also contributed to the development and implementation of the Resident Quality Inspection process in Ontario.


DonnaDonna Cheatham, RN, MS
National Account Services Manager

With over 30 years of experience as a registered nurse, Donna brings extensive clinical, educational, and regulatory knowledge to Providigm as a member of the National Account Services team. Prior to joining Providigm, she worked for both the Ohio and Florida state survey agencies in multiple roles including surveyor, QIS project lead, quality assurance coordinator, and state training coordinator.


TraciTracy Cooley, RN, BSN
Senior National Account Services Director

As a registered nurse, Tracy has held numerous positions within long-term care including: director of nursing, assistant director of nursing, restorative nurse, assessment coordinator, and staff nurse. Tracy served as a registered nurse health care surveyor with the Ohio Department of Health for five years, and became a Registered QIS Surveyor in February 2006. While at the Ohio Department of Health, she conducted numerous QIS surveys and functioned as a supervisor of a QIS survey team.  In 2009 Tracy became a Master QIS Trainer with Providigm, working with the state survey agencies in the implementation of the QIS survey process.

TamiTami Green, RN
Director of Education

Tami brings a wide array of clinical and regulatory experience to the educational experience of providers. As a registered nurse, she has worked in long-term care as a nurse supervisor, quality assurance nurse, director of nursing, and clinical research nurse. After gaining experience in long-term care, Tami became a surveyor with the State of Ohio. During her five-year tenure as a surveyor, she was selected to join one of the first teams to participate in the QIS demonstration project and later went on to became a registered QIS surveyor and state QIS trainer. In 2009 Tami joined Providigm and became a Master QIS Trainer, working with state survey agencies during the roll-out of the QIS survey process.

RobinRobin Harper RN
Senior Director of Training

Robin has been a registered nurse for over 30 years, and has extensive experience in long-term care and regulatory compliance and guidance. Robin served as a state surveyor for the Ohio Department of Health for more than eight years, and was selected to participate on Ohio’s first QIS pilot team prior to the national implementation of the QIS process. In 2007, she joined Providigm and became a Master QIS Trainer and Director of Training under Providigm’s QIS training contract. Robin is currently Providigm’s Director of Training and works with a team of exceptional trainers who assist long-term care providers with implementation of the abaqis Quality Management System. Robin believes we all must work together to make a difference in the long-term care field, and she feels fortunate to be able to work with providers who share her passion for ensuring that all nursing home residents receive the highest quality of care.

JaneJane Hermanson, RD, MBA
Master QIS Trainer

Jane brings a wide range of regulatory and clinical experience in long-term care to her role as a Providigm trainer.  She is a Registered Dietitian and worked for the State of Colorado’s Department of Public Health and Environment for 15 years, first as a surveyor and later as a Program Manager in long-term care.  When the QIS debuted, Jane transferred to the University of Colorado’s Division of Health Care Policy and Research to join the QIS roll-out team.  The pilot evolved into a national roll-out and Jane joined Providigm as the Director of Training Development during the multi-state roll-out of the QIS process.

Chief Sales and Marketing Officer

Ellen has seen the healthcare industry change more dramatically in the last 10 years than it has in the last 50. With over 12 years of expertise in advising and marketing to government-regulated medical industries, including the medical device, tissue transplant, biologic, pharmaceutical and long-term care industries, she is able to help acute and post-acute clients to improve clinical outcomes and increase revenue.


CindyMCindy Mason
Vice President of Provider Services

Cindy’s professional background, which consists of over 30 years of advising nursing home administrators and staff, makes her ideally suited to assist nursing homes in assessing and refining systems for survey success. Cindy provides education and consultation to nursing home providers. Licensed as a clinical social worker, nursing home administrator, healthcare risk manager, and accredited as a QIS instructor, she translates complex regulatory requirements into concepts that can be applied to managing the daily operations of nursing home life. As a long-term care consultant, staff educator, and national speaker, Cindy has worked with all levels of nursing home staff on regulatory compliance and skill development. Cindy has worked extensively with consumers of care whose perspective is critically important in survey success.

KevinKevin O’Connor
Director of Sales

Kevin has been involved in sales and operations management for over 16 years; 11 specifically in the healthcare arena. As an administrator in the surgical world, Kevin helped grow and streamline operations in the LASIK industry across the mid-Atlantic region. He would then spend 7 years consulting with leading hospitals and private practice organizations on everything from recruitment and operations to team-building and retention strategy.

SebrenaSebrena Easley Sweeney RN, BSN
Account Management Director

Sebrena is a registered nurse and has served as a Master QIS Trainer with Providigm since August 2010. She began her long-term care career in 1999 as an MDS coordinator. She then went on to become a facility survey consultant for the State of North Carolina for seven years. Sebrena has also worked as a senior regional nurse consultant for two long-term care corporations and led multiple facilities to deficiency free surveys. Her areas of expertise include quality improvement and regulatory compliance, as well as nursing policy and procedure.

JeanneanJeannean Turner
abaqis Trainer

Jeannean has been a registered nurse for more than 25 years and has extensive experience in both acute clinical care and long-term care regulatory compliance. Prior to joining Providigm, she worked for 16 years at the Ohio Department of Health where she held such positions as long-term care surveyor, infectious disease control consultant, assistant district office supervisor, and field manager. While with the Ohio Department of Health, Jeannean’s responsibilities included planning, coordinating, and overseeing quality assurance survey activities related to long-term care state licensure and federal certification requirements. In this role, she also guided the implementation of the QIS process and served as a resource for surveyors conducting QIS surveys. As our newest addition to the Providigm training team, Jeannean is excited to assist long-term care providers in achieving their quality assurance goals.
